Please note: The sample forms listed below must be reviewed with consideration for the particular program/activity and supplemented as necessary. Before making changes, please consult with the Policy Owner by emailing: programsforminors@uga.edu.

This form is the Agreement, Waiver, and Media Release for Online Programs & Activities

This form is for parental consent for Online Programs & Activities.

This guide should be used by all Programs & Activities moving to online programs during the current health situation.

Guideline on Two-Adult Rule

All Program Staff, including volunteers, must agree to abide by a Staff Code of Conduct and receive appropriate training on its requirements. 

Program Administrators may utilize the Program Checklist to ensure their program/activity is in compliance with the Policy. 

Required form from Approving Official for Third Party Programs/Activities

All minor program/activity participants and their parent/guardian should complete the Pick Up Authorization.

All minor program/activity participants and their parent/guardian should complete the Participation Agreement, Waiver, and Media Release. Program Administrators should supplement this form with program-specific information and submit to the Office of Legal Affairs (legal@uga.edu) for approval prior to obtaining signatures.

All minor program/activity participants and their parent/guardian should complete the Participant Code of Conduct.
