This page is intended to assist Program Administrators and University Departments/Units sponsoring a program/activity.
Please note: The sample forms listed below must be reviewed with consideration for the particular program/activity and supplemented as necessary. Before making changes, please consult with the Policy Owner by emailing: programsforminors@uga.edu.
All Program/Activity Staff must complete a Staff Code of Conduct.
Program Administrators may utilize the Program Checklist to ensure their program/activity is in compliance with the Policy.
All minor program/activity participants and their parent/guardian should complete the Pick Up Authorization.
All Programs/Activities should have a consent form; this version is approved by UGA Legal Affairs. If using a different version you must get approval from UGA Legal Affairs.
All minor Program/Activity participants and their parent/guardian should complete the Participant Code of Conduct.
All minor Program/Activity participants and their parent/guardian participating in residential Programs/Activities should complete the Medical Information Form and Authorization for Medical Care. Note that the University Health Center does not treat pediatric urgent care, so programs should plan for off campus urgent care centers or emergeny rooms for care; a copy of the student’s health insurance card is typically required to treat. No changes should be made to this form.
Deans, Vice Presidents, Directors, and Department Heads of University Departments/Units sponsoring Programs and Activities may utilize the Checklist for Approving Third Party Programs to determine whether to approve the use of University facilities for a third party Program/Activity. Each third party is responsible for completing a Facilities Use Agreement and certifying compliance with the Policy.
All minor Program/Activity participants and their parent/guardian should complete the Authorization to Administer Medication. This form is typically only needed for residential Programs. No changes should be made to this form.
