This form is the Agreement, Waiver, and Media Release for Online Programs & Activities
This form is for parental consent for Online Programs & Activities.
This guide should be used by all Programs & Activities moving to online programs during the current health situation.
All Program Staff, including volunteers, must agree to abide by a Staff Code of Conduct and receive appropriate training on its requirements.
Program Administrators may utilize the Program Checklist to ensure their program/activity is in compliance with the Policy.
All minor program/activity participants and their parent/guardian should complete the Pick Up Authorization.
All minor program/activity participants and their parent/guardian should complete the Participation Agreement, Waiver, and Media Release. Program Administrators should supplement this form with program-specific information and submit to the Office of Legal Affairs (firstname.lastname@example.org) for approval prior to obtaining signatures.
All minor program/activity participants and their parent/guardian should complete the Participant Code of Conduct.
All minor program/activity participants and their parent/guardian should complete the Medical Information Form and Authorization for Medical Care. If your program plans to utilize the University Health Center, their policy requests this signed form and a copy of the student’s health insurance card to treat. Deans/Directors/Vice-Presidents of University Departments/Units sponsoring programs and activities may utilize the Checklist for Approving Third Party Programs to determine whether to approve the use of University facilities for a third party program/activity. Each third party is responsible for completing a Facilities Use Agreement and certifying compliance with the Policy.
