This page is intended to assist Program Administrators and University Departments/Units sponsoring a program/activity.

Please note: The sample forms listed below must be reviewed with consideration for the particular program/activity and supplemented as necessary. Before making changes, please consult with the Policy Owner by emailing: programsforminors@uga.edu.

All Program Staff, including volunteers, must agree to abide by a Staff Code of Conduct and receive appropriate training on its requirements. 

Program Administrators may utilize the Program Checklist to ensure their program/activity is in compliance with the Policy. 

All minor program/activity participants and their parent/guardian should complete the Participation Agreement, Waiver, and Media Release. Program Administrators should supplement this form with program-specific information and submit to the Office of Legal Affairs (legal@uga.edu) for approval prior to obtaining signatures.

All minor program/activity participants and their parent/guardian should complete the Participant Code of Conduct. 

Deans/Directors/Vice-Presidents of University Departments/Units sponsoring programs and activities may utilize the Checklist for Approving Third Party Programs to determine whether to approve the use of University facilities for a third party program/activity. Each third party is responsible for completing a Facilities Use Agreement and certifying compliance with the Policy.

All minor program/activity participants and their parent/guardian should complete the Pick Up Authorization.
